A Hold Point generally means that a specific inspection, verification, review, or approval must be completed before the work can proceed to the next stage.

Welding → Inspection → Hold Point → Next Activity

If the required party has not completed the inspection or provided the required release, the work should not proceed past that defined point.

Hold Points are particularly important because proceeding without the required inspection or release could make verification more difficult or, in some cases, impossible later.

For example, once a component is covered, insulated, coated, backfilled, or otherwise made inaccessible, performing certain inspections afterward may not be practical.

This is why QC Inspectors should identify Hold Points before the work reaches that stage.


6. Understanding Witness Points

Another common inspection point is the Witness Point.

A Witness Point generally provides a designated party with the opportunity to witness an inspection or test.

Unlike a Hold Point, the work may be permitted to proceed according to the project’s defined requirements if the witnessing party does not attend after the appropriate notification and waiting requirements have been satisfied.

However, the exact rules can vary by project.

This is why inspectors should always follow the project’s approved ITP, procedures, and contractual requirements rather than assuming that every Witness Point operates the same way.


7. Review Points and Surveillance

Some ITPs may also identify other types of inspection or verification points.

These can include:

Review Points

A document, record, procedure, drawing, or test result may need to be reviewed.

Surveillance

The QC team or client may monitor an activity without requiring a formal release before work continues.

Inspection Points

A specific inspection is performed to verify compliance with the defined requirements.

The terminology and abbreviations can vary between organizations.

Some projects use codes such as:

  • H – Hold Point
  • W – Witness Point
  • R – Review
  • S – Surveillance

Always check the project’s ITP legend or definitions.


8. Who Is Responsible?

Step 4: Review the Applicable References

Know which codes, specifications, drawings, procedures, and acceptance criteria apply.

Step 5: Confirm Documentation Requirements

Know which reports, checklists, certificates, and records must be generated.

Step 6: Coordinate With the Project Team

If client or third-party involvement is required, make sure the appropriate notifications are made according to the project requirements.

Step 7: Maintain Traceability

Make sure inspection records can be connected back to the relevant:

Common Mistakes When Using an ITP

Even experienced inspectors can run into documentation problems if the ITP is not reviewed carefully.

Missing an Inspection Point

The work progresses before the required inspection is completed.

Using the Wrong Acceptance Criteria

The inspector references the wrong specification or standard.

Missing Required Documentation

The inspection is completed, but the required report or record is not properly documented.

Poor Traceability

The report does not clearly identify the weld, component, material, drawing, or location inspected.

Assuming All Projects Use the Same ITP
